New Patient Registration
Please pick up a registration pack from reception which gives details of the proofs of ID and residence that you will need to show us.
The main documents contained in the pack are downloadable below if you'd like to print them out at home, fill them in and bring them, together with the proofs of identification and residence, to the surgery.
A Sharing Choices Form (downloadable below) is an important part of the registration procedure as it makes clear your consent - or not - to share various aspects of your patient record with other organisations outside the practice.
We have been a Summary Care Record practice for some years and most of our patients have consented to share it with other NHS hospitals, Out of Hours Centres and GP surgeries elsewhere in the country. The Summary Care Record consists of your name and contact details, plus your medications and any adverse drug reactions or allergies that have been recorded on your medical record.
There is also the opportunity to share a larger part of your medical record with NHS and other organisations including the ambulance service, Gloucestershire Care Services and Gloucestershire County Council social care as part of the JUYI or Joining Up Your Information project.
From April 2018, in line with NHS Gloucestershire Clinical Commissioning Group policy, we have made our patient records shareable via the electronic Data Sharing Module or eDSM.

Registration administration takes several days after we have received your signed forms. If you require urgent medication, remember your existing surgery has a duty of care for you until we receive your medical records.
PLEASE PRINT OUT AND COMPLETE ALL OF THE FORMS BELOW AND BRING THEM WITH YOUR ID TO THE SURGERY (preferably after 10.30am)

Out of Area Patients:
We accept registration applications from patients living outside our practice area who have good reason to be registered away from home.
Please be aware that Out of Area registration is not guaranteed. Each application will be considered by the doctors on an individual basis and decisions made in the best clinical interests of the patient. For more information about Out of Area patient status click here
